- The patient is morbidly obese. Everybody can see that he needs to lose weight. But he just doesn't want to, and threatens that if his calories are cut, he'll stop eating health food. (No mention of all the junk he's currently consuming).

- This bioethicist needs to do his research. Obesity is a metabolic disorder, caused by eating a high carbohydrate diet. If the government / media ever stop encouraging people to eat low fat, high carbohydrate foods, the epidemic will end.
